The Out of Hours (OOH) Care Community incorporates the OOH GP team, nursing team, allied health professionals and voluntary services.
The OOH Nursing team work between 17.00 and 08.00, seven days a week, and cover the whole Community of East Cheshire Trust, from their office in Macclesfield Hospital. The team consists of District Nurses, Community staff nurses and Healthcare Assistants. Referrals are accepted from Health care professionals and health and social care agencies, as well as directly from patients and relatives.
The OOH Nursing Team provide a rapid response service for patients in an evening and over night, specialising in end of life symptom management and palliative care, as well as bladder and bowel care, complex wound care and medication administration and support.
